Job Responsibility:
Social prescribing empowers people to take control of their health and wellbeing through referral to non-medical link & well being workers. The Link & Well Being Worker will support existing groups and help people start new community groups, working collaboratively with local partners.
Social prescribing can help strengthen community resilience and personal resilience, reducing health inequalities by addressing debt, poor housing, and physical inactivity. It works for people with long-term conditions, mental health support, and those who are lonely or isolated.
The Social Prescribing Team works across a diverse range of community groups, including Elderly, Youth, and those with a Learning Disability. Key responsibilities include:
- Taking referrals and managing caseload
- Undertaking holistic assessments and developing tailored Personal Action Plans
- Reducing health inequalities and proactively working to reach people from all communities
- Developing relationships with local communities and service providers
- Working with partners to ensure sustainable community groups and assets
- Advocating for the Social Prescribing project
- Being flexible and person-centred
Key behaviours include empathy, accuracy, inter-personal skills, adaptability, resilience, organisation, and analytical skills. The role requires the ability to communicate effectively, provide leadership, and make decisions.
Salary: Dependent on skills and experience. Reporting line: The role reports to the Senior Social Prescriber.
