Role
As a Support Practitioner you will assist with all aspects of daily living including meal preparation, medication, appointment support and activities, personal finance management, mobility support and personal care. The individuals we support have diverse needs, including mental health challenges, learning disabilities, addictions, complex needs and behaviours of concern.
Core Responsibilities
- Assist clients with meal preparation and medication management.
- Support clients in attending appointments and taking part in activities.
- Help with personal finance and budgeting.
- Provide assistance with mobility and handling, including driving where required.
- Offer personal care and hygiene support.
- Encourage active engagement in hobbies and social connections to reduce isolation.
- Maintain accurate records and communicate with the team regarding client needs.
Essential Requirements
- Full UK manual driving licence with a minimum of 1 year’s driving experience and access to a vehicle.
- Empathetic and caring nature.
- Excellent verbal and written communication skills.
- Confidence in using computers.
- Ability to work well within a team and independently.
- Good organisational skills.
Shift Patterns
Shifts include 24/7 support, night shifts, early starts from 7:30 am, and back shifts until 8–9 pm. Work is scheduled 5 days over a 7‑day period and includes weekend and public holiday rotations.
