Job Summary: Support high quality rehabilitative palliative care for patients of Phyllis Tuckwell Hospice. The Rehabilitation Assistant will work across outpatient, group settings and the community, providing interventions that enable patients to manage their condition and engage in meaningful activities.
Responsibilities:
- Assist in delivering person‑centred, high‑quality rehabilitative care to patients, carers and families.
- Work within a multidisciplinary team to assess patients’ needs and develop individualized interventions.
- Use a range of therapeutic techniques to improve physical, emotional and social wellbeing.
- Encourage patients to maximise functional abilities and promote quality of life.
Qualifications & Experience:
- Level 3 qualification in Health or Social Care (or equivalent).
- Understanding of palliative care principles.
- Committed to person‑centred, rehabilitative care for patients and carers.
- Experience working as part of a multidisciplinary team.
- Well‑developed communication skills.
- Competence with information technology across a range of platforms.
- Self‑awareness and resilience.
