Salary: £46,920 per annum WTE (based on 37.5 hours a week)
Flexible working opportunities available. Hours work across 4 days of 9.5 hours (38 hours) or 5 days of 7.5 hours (37.5 hours) on a seven‑day rota.
We are looking for a passionate nurse with experience in palliative and end‑of‑life care to join our community team and develop into a Clinical Nurse Specialist. The role involves providing specialist palliative care to patients, their families and carers in the community, working as part of a multi‑professional team and supporting patients to live well.
Key responsibilities include:
- Developing advanced communication skills and advance care planning
- Managing symptom management and caseload effectively
- Providing responsive care and working with multidisciplinary teams around the patient
- Advancing clinical practice, sharing knowledge and educating others
- Continuously improving services and undertaking further education such as MSc modules in Advanced Clinical Practice
Qualifications:
- RGN Degree Level Qualification
- Current NMC registration in adult or general nursing
- Palliative/end‑of‑life care or long‑term conditions experience
- Car driver and owner
- Experience of multidisciplinary team working and with external partners
- Able to make autonomous decisions demonstrating decision‑making, critical thinking and proactive caseload management
