We are seeking a highly motivated and experienced Clinical Pharmacist Independent Prescriber (IP) to work in a hybrid capacity across two GP practices – Dearden Practice and The Gill in Manchester. The successful candidate will play a key role in delivering high‑quality, patient‑centred clinical pharmacy services, supporting both practices with medication optimisation, long‑term condition management, and independent prescribing responsibilities.
- Deliver clinical pharmacy services across both GP practices, ensuring safe, effective, and evidence‑based prescribing.
- Conduct structured medication reviews (SMRs), particularly for patients with long‑term conditions and polypharmacy.
- Act as an Independent Prescriber, managing patient caseloads within scope of competence.
- Support management of chronic diseases such as hypertension, diabetes, asthma, and COPD.
- Provide medication optimisation advice to patients and the multidisciplinary team.
- Manage repeat prescriptions, medication queries, and reconcile medicines following hospital discharge.
- Work collaboratively with GPs, nurses, and wider PCN teams.
- Use EMIS Web to document consultations, prescribing, and clinical interventions.
- Maintain compliance with NHS policies, governance, and safety standards.
- Support QOF, audits, and other quality improvement initiatives.
