We are looking to recruit an enthusiastic and motivated Care Navigator to join our fantastic team at The Pendle Medical Partnership. If you thrive on helping people, are highly organised, motivated, comfortable with technology, have strong customer service skills and the desire to assist patients in accessing the services they need, this is the role for you.

You will be based across our Earby & Colne sites on the following working pattern:

  • Monday 13:00 - 18:00
  • Wednesday 08:00 - 13:00
  • Thursday 13:30 - 18:30
  • Friday 08:00 - 17:00
  • Some Saturday mornings throughout the year are required.

Experience of using EMIS Web and working in a GP Practice, healthcare setting or in customer services is preferred but full training and support will be given to the successful candidate.

The successful candidate will work as a member of our Care Navigation team. This is a varied role that can be challenging and rewarding, but different every day. You will be required to undertake a variety of tasks that support our patients and clinicians who deliver the best care to our patients. The role includes dealing with patient enquiries, either in person, via our eConsult system or by telephone, navigating patients to the most appropriate healthcare professional whilst utilising additional services, handling computerised patient information as well as providing general assistance to the wider practice team.
