Responsibilities

  • Deliver Direct Patient Care.
  • Make professional, autonomous decisions in relation to presenting problems, whether self‑referred or referred from other health care workers within the organisation.
  • Screen patients for disease risk factors and early signs of illness.
  • Cover all the clinical management of the patients in surgery as appropriate, including initiating investigations, reviewing results, and making referrals to secondary care or to other providers as appropriate.
  • Develop care and treatment plans in consultation with patients and in line with current Practice disease management protocols and wider guidance such as that from NICE.
  • Ensure appropriate follow‑up of patients.
  • Record clear and contemporaneous computerised consultation notes to agreed standards.
  • Participate in clinical audits and learning event analyses and practice meetings.
  • Compile and issue computer‑generated acute and repeat prescriptions, avoiding hand‑written prescriptions.
  • Prescribe generically whenever appropriate and in accordance with the Practice prescribing formulary, NICE guidance and Good Prescribing Practice (as defined by British National Formulary guidance).
  • Work with and provide support for other members of the primary health care team.
  • Play a lead role in the practice achieving agreed and other key performance indicator targets.

  • Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities, and provide evidence of learning activity as required.
  • Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, equality, diversity and inclusion training, and health and safety.

Miscellaneous

  • Establish strong working relationships with GPs and practice teams and work collaboratively with other staff of the multi‑disciplinary team, supporting each other, respecting each other’s views and meeting regularly as a team.
  • Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner.
  • Contribute to the wider aims and objectives of the PCN to improve and support primary care.
