Experienced Practice Nurse

Park Medical Practice is looking to recruit an experienced and enthusiastic Practice Nurse to join our established nursing team, working 26 to 28 hours per week across our Chaddesden, Borrowash and University of Derby sites. This is a varied General Practice nursing role, providing high quality, evidence based care to our patient population. The successful candidate will have the opportunity to work across a broad range of clinical areas, including long term condition management, treatment room services, cervical screening, childhood and adult immunisations, travel health and preventative healthcare. You will work as part of a supportive multidisciplinary team, with day to day clinical leadership provided by our Clinical Nurse Manager. We are looking for a nurse who is patient focused, professional, organised and committed to maintaining high standards of clinical care.

The successful candidate will manage their own clinical workload and provide a broad range of nursing services, working collaboratively with the wider Practice team to support safe, effective and coordinated patient care. A key part of the role will be the assessment and ongoing management of patients with long term conditions, including diabetes, asthma, COPD, hypertension, chronic kidney disease and cardiovascular disease. The post holder will also contribute to treatment room services, screening and immunisation programmes, travel health and health promotion. The Practice Nurse will support the achievement of QOF and other Practice priorities through high quality clinical care, accurate record keeping and effective use of SystmOne. The role also includes contributing to clinical governance, audit, infection prevention and control, patient safety and quality improvement, while maintaining professional standards in accordance with the NMC Code.

About us: Park Medical Practice is a large training GP practice providing primary care to approximately 26,500 patients across our Chaddesden, Borrowash and University of Derby sites. We have a workforce of around 90 staff and a diverse multidisciplinary team working together to provide safe, effective, responsive and compassionate care to our patients. Our nursing team is led by a dedicated Clinical Nurse Manager and includes a Team Lead Nurse, Practice Nurses and Healthcare Assistants. The successful candidate will work alongside GPs and the wider clinical, administrative and management teams, with a strong emphasis on teamwork and collaborative working. The Practice Nurse will deliver high quality, evidence based nursing care across all three Practice sites. The post holder will provide treatment room services, long term condition management, cervical screening, travel health, immunisations and preventative healthcare, while contributing to QOF achievement, clinical governance, patient safety and continuous quality improvement. The post holder will work collaboratively with the Clinical Nurse Manager, GPs and wider multidisciplinary team to promote health, wellbeing and self management and to support the achievement of Practice objectives and enhanced services.

• Deliver evidence based long term condition reviews including diabetes, asthma, COPD, hypertension, chronic kidney disease and cardiovascular disease.
• Provide treatment room services including wound care, dressings, suture removal, venepuncture, ECGs, spirometry and NHS Health Checks.
• Undertake cervical screening in accordance with national guidance.
• Administer childhood, adult, seasonal and travel vaccinations in accordance with appropriate PGDs and PSDs.
• Provide health promotion, lifestyle advice and preventative healthcare.
• Maintain accurate patient records and clinical coding within SystmOne.
• Recognise deteriorating patients and elevate concerns appropriately.

The post holder will support achievement of QOF indicators through the delivery of high quality clinical care. Use approved clinical templates and appropriate coding to ensure accurate data capture. Support effective patient recall systems and appropriate follow up of patients. Work collaboratively with the wider Practice team to support long term condition management and Practice objectives.

The Practice Nurse will participate in clinical audit and quality improvement activity. Comply with CQC standards and Practice requirements relating to safeguarding, infection prevention and control and information governance. Contribute to learning arising from complaints, incidents and Significant Events. Maintain high standards of patient safety and professional practice.

The post holder will maintain high standards of infection prevention and control within treatment rooms and clinical areas. Follow cold chain procedures and requirements for safe vaccine management. Identify and report risks appropriately. Participate in and support infection prevention and control audits.

The Practice Nurse will maintain current NMC registration and meet all requirements for professional revalidation. Work in accordance with the NMC Code and within their individual scope of professional competence. Participate in mandatory and statutory training. Maintain continuing professional development appropriate to the role. Comply with Practice policies, procedures and clinical protocols.

The post holder will use clinical resources responsibly and appropriately. Monitor clinical stock and report shortages or concerns. Support the Practice in achieving value for money and reducing unnecessary waste.

The Practice Nurse will maintain effective professional working relationships with GP Partners, the Practice Manager, Clinical Nurse Manager, nursing colleagues, Healthcare Assistants, reception and administration teams, PCN colleagues, community services, secondary care providers and patients. The post holder will communicate effectively with patients, carers and colleagues and contribute positively to multidisciplinary and team working across the Practice.

The post holder will comply with all Practice policies and procedures. Maintain patient confidentiality and comply with UK GDPR and information governance requirements. Promote equality, diversity and inclusion. Participate in annual appraisal and maintain an appropriate personal development plan. Maintain the professional standards expected of a Registered Nurse. Undertake any other reasonable duties commensurate with the role.

Qualifications:

• Essential: Registered Nurse with current NMC registration, recognised cervical screening qualification/competency, recognised immunisation and vaccination training/competency, relevant post-registration training in long term condition management.
• Desirable: Independent Nurse Prescriber qualification, Travel Health qualification or recognised training, additional post-registration qualifications relevant to General Practice nursing.

Experience:

• Essential: Experience of working as a Practice Nurse within General Practice or Primary Care, experience of long term condition management, including diabetes, asthma, COPD, hypertension, chronic kidney disease and cardiovascular disease, experience of cervical screening, experience of childhood and adult immunisations, experience of treatment room services, including wound care, venepuncture and ECGs, experience and competence in using SystmOne, including clinical documentation, templates and coding, experience of supporting QOF achievement and effective patient recall, experience of health promotion and preventative healthcare, ability to work independently and manage an appropriate clinical workload.
• Desirable: Experience of travel health immunisations, experience of clinical audit and quality improvement, experience of working across multiple Practice sites.
