Essential Duties and Responsibilities:
- Perform comprehensive patient assessments to determine functional deficit and potential and establish a therapeutic program.
- Develop a therapy plan of care to include goals and discharge plans.
- Document, within time frames, observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition.
- Complete all documentation required and in accordance with state and federal regulations and policy.
- Demonstrate administration of objective, standardized tests and measures and performance of evidence-based practice in documentation.
- Communicate and coordinate patient care with other members of the patient's health care team.
- Attend employee meetings and patient conferences.
- Supervise physical therapy assistants in accordance with regulations and policy.
- Provide patient and family teaching related to the patient's physical therapy program and patient/family engagement in goal achievement.
- Assist the patient and/or family to procure needed equipment or supplies.
Position Requirements:
- Graduate of an accredited Physical Therapy program
- Licensed as a PT in Massachusetts
- Home care experience strongly preferred.
- Current CPR required.
- Current State of Residence driver's license and reliable transportation.
