Principal Duties / Essential Functions:
- Monitor and complete workflow tasks in HCHB
- Code commercial OASIS
- Organize educational programs based on needs
- Review clinical records for completeness
- Collaborate with supervisory staff for documentation accuracy and effectiveness
- Lead case conferences and staff meetings
- Educate staff on audit deficiencies
- Evaluate clinical competency of nursing staff
- Participate in orientation of new team members
- Perform Blood Pressure clinics as needed
- Provide direct skilled nursing care in the field as per physician orders
Supervisory Responsibilities:
- Serve as Home Care EMR resource with extended access and roles
- Supervise branch staff, conduct annual reviews, and orientations
- Train new clinicians on HCHB and homecare regulations
- Review orders and oversee care as per licensure
Qualifications:
- Graduate of an accredited nursing or physical therapy school; BSN preferred, MSN strongly preferred; master's in physical therapy preferred; home care experience preferred
- Minimum 3 years’ managerial experience
- Minimum 5 years’ patient care experience
- Current RN or Physical Therapist license in Massachusetts
- CPR certification required
