Base pay range: $17.00/hr - $24.00/hr

POSITION DUTIES/ESSENTIAL FUNCTIONS:
- Assist patients with bathing, toileting, dressing, shampooing, feeding, shaving, mouth care, etc.
- Assist with simple exercise programs, application of prostheses and braces, ambulation and transfer activities (including use of assistive devices) and accompanying patient out of doors.
- Assist with medications normally self-administered or remind patient as directed by the primary care nurse.
- Planning, shopping and preparing meals accordingly to prescribed diet.
- Light housekeeping activities, such as changing bed linens, laundry and cleaning as necessary to maintain a healthy environment.
- Accompany patients to the physician or clinic, etc., when part of the care plan and approved by direct supervisor.
POSITION REQUIREMENTS:
- High school diploma or equivalent - required
- Valid Home Health Aide Certificate - required
- Current MA Driver's License and reliable transportation
- Successful completion of the Home Health Aide Competency Exam with a minimum overall score of 80% and minimum 80% in each section - required
