This role invites a qualified Family Care Practitioner to work across the three sites in the Fireclay and Old School Surgery (FOSS) Primary Care Network. You will deliver autonomous clinical assessment and care pathways on a flexible schedule of up to 37.5 hours per week.
Responsibilities
- Act as a highly specialised practitioner within the GP practices, performing clinical assessment and formulating care pathways in place of a medical GP.
- Provide advanced musculoskeletal assessment as the first point of contact, diagnosing and advising on management, and referring appropriately, including liaising with GPs for patients with red flag or non‑musculoskeletal presentations.
- Arrange relevant clinical investigations (e.g., X‑rays, pathology tests) to support accurate diagnosis and effective condition management.
- Collaborate closely with GPs, GP Practice Team members, MSK services, and MSK Physiotherapy outpatient services to optimise patient care.
- Work across all three practices (Old School Surgery, Lodgeside & St George) to promote integrated, patient‑centred care within a multidisciplinary team.
Qualifications
- Extensive prior experience in a primary or family care setting.
- Proven ability to handle a high volume of diverse patients and multidisciplinary collaboration.
- Strong clinical assessment skills and capacity to manage complex musculoskeletal cases independently.
