Nursing Assistant

We have a fantastic opportunity for a Nursing Assistant to join our Myton at Home team. The Myton at Home service provides expert and compassionate care to patients in their own homes, at a crucial time in their lives. This role involves home visits within the Warwickshire area. You will be based at our Rugby Myton Support Hub, but you may also be required to work at our Warwick or Coventry Hospices on occasion.

• NVQ level 3 (or equivalent), or willingness to work towards this (essential)
• UK driving licence

If you’re offered the role, you will need an enhanced DBS check, including the children's and adult's barred list, because you will be working in regulated activity with patients and their families. We will arrange and cover the cost of this check.

Job responsibilities include:

• Supporting the nursing team in the delivery of nursing care to the patients and their family/caregivers, in their own home, providing evidence-based, individualised end of life care.
• Supporting the Primary Healthcare teams in the delivery of its services, in line with the District nurses care plan, whilst adhering to statutory regulations and Hospice policies.
• Working under the direct and indirect supervision of the Hospice at Home Lead, in the delivery of quality holistic nursing care for patients in their own homes.
• Responding to the specific needs of palliative care patients and their significant others, in line with agreed care plans.
• Recognising and reporting any changes in patients' condition and where appropriate providing care and support under the direction of a Registered Nurse.
• Communicating effectively and sensitively to patients, their significant others and other professionals, referring questions and concerns to the appropriate personnel.
• Assessing and reassessing patients' capabilities as needs change regarding assistance with the activities of daily living and emotional care.
• Ensuring safe and responsible use of equipment, identifying and reporting any malfunction, and taking action to maintain safety as directed.
• Providing practical support to the family around safe delivery of personal care; assisting with last offices and supporting other nursing staff with the care of the deceased where appropriate.
• Promoting and maintaining a safe environment, dignity, and privacy at all times.
• Meeting the physical, emotional, spiritual, and social needs of the patient and family; contributing to a team-based approach to care; managing time and resources effectively.
• Maintaining standards and confidentiality in line with Caldicott Guidelines; being approachable, non-judgemental, trustworthy, and motivated to deliver high quality care.
• Being able to work autonomously or as part of a team; flexible and professional to meet service needs; creating and maintaining accurate contemporaneous patient records; complying with Myton Hospice policies.
• Complying with Health and Safety requirements; contributing to service development; being responsible for own continued development; managing clinical duties and resources; lone worker policy adherence; ensuring transport is road-worthy as per policy.

Person Specification:

• Two years' experience of hospice, hospital or community nursing
• Experience of Multi-disciplinary Team working
• Experience working in a palliative care setting
• Experience of caring for patients and families with life limiting conditions / life threatening conditions

Qualifications:
