Watergate Primary Care Network is recruiting a Work and Health Coach for a fixed-term role to support patients signed off work. You will hold workability conversations and co-produce personalised plans that replace standard fit notes, coaching patients back to or into work.
You will work with GPs and occupational therapists within a MDT, contributing to the national pilot, collecting data and evaluating outcomes to improve the service.
