The Methodist Church in Britain is seeking a Health in Justice social prescribing link worker to support HMP Wakefield, HMP Wealstun, and HMP Leeds. On-site role covering all prisons, with vetting checks prior to offer. You will link detainees to activities and services to improve wellbeing and reduce medication reliance.
You will assess needs, develop personalised plans, manage a caseload, and liaise with prison and health teams to ensure coordinated, whole-system support.
