The Confederation, Hillingdon CIC is seeking a GP to provide senior clinical oversight within the Reactive Care Coordination Hub for a one-year fixed-term post. You will perform rapid assessments of urgent referrals, decide on pathways to avoid admission and support a two-hour morning handover, ensuring coordinated care across UCR, Virtual Ward, D2A and YLL services.
Applicants must hold GMC registration and be a qualified GP, with extensive experience in urgent care and virtual consultations.
