Cornwall Partnership NHS Foundation Trust is seeking an experienced Programme Manager to lead the neighbourhood frailty programme from design through to evaluation across 16 Integrated Neighbourhood Teams in Cornwall and the Isles of Scilly.
You will manage a £6.8 million non-recurrent budget, build partnerships across primary care, community and acute services, and translate national priorities into practical delivery plans while ensuring governance, risk management and benefits realisation.
