Julia's House is seeking a Transition Nurse to oversee a caseload of children and young people (around age 14+) as Named Nurse. You will coordinate person-centred transition plans and work across hospice, home and community settings, including travel to schools, colleges and hospitals.
The role requires active NMC registration (Child Branch or Learning Disability/Adult) and experience with complex needs, safeguarding and MCA/DoLS guidance.
