Wye Valley NHS Trust is recruiting a Band 6 Therapy Clinical Decision Maker to the Single Point of Access team at its Community Referral Hub. The post centres on clinical assessment, triage and decisions about which community service a therapy referral should go to, with the post holder judging urgency and risk. The work is mostly by telephone and sits within a multidisciplinary decision-making team alongside a senior triage clinician and a hub GP. It suits an experienced registered clinician who is confident making autonomous risk decisions remotely.

What the job involves

  • Undertake comprehensive, mostly telephone-based clinical triage and assessment of patients with therapy needs, across a wide range of acute, complex and changing needs
  • Triage and screen urgent and routine therapy referrals, filtering them to the UCR and Neighbourhood team and identifying the onward service
  • Work within an MDT clinical decision-making team with a senior triage clinician and hub GP, using clinical reasoning and evidence-based practice to set urgency, risk and course of action
  • Recognise acute deterioration, red flags and safeguarding concerns and take immediate, appropriate action
  • Work across organisational and professional boundaries with community teams, primary care, ambulance services, acute providers, social care and system partners to coordinate the right pathway of care
  • Provide leadership in clinical decision-making for therapy referrals
  • Contribute to developing, implementing and reviewing clinical pathways, triage frameworks, protocols, standard operating procedures and service guidance
