Avon & Wiltshire Mental Health Partnership NHS Trust is seeking a Recovery Coordinator for the North Wiltshire Intensive Team. The role covers home treatment and assessment as an alternative to admission, with seven-day cover and some lone working. Visa sponsorship is not available.
You will organise assessments, plan and review care, and work with service users and carers on recovery plans, risk management and discharge planning, while maintaining confidentiality and accurate records.
