A part-time Registered Nurse post on a 12-month secondment or fixed-term basis within the Swadlincote Community Nursing Team at Derbyshire Community Health Services NHS Foundation Trust. The nurse assesses and plans individual care in patients' own homes, promoting self-care and working alongside general practice and therapy colleagues. The role also involves guiding junior staff, covering weekends and bank holidays, and driving across a wide area with equipment. Flexible hours may be considered for the right candidate, and RGN registration is required.
What the job involves
- Assess, plan, implement and evaluate individualised patient care
- Promote self-care while maintaining privacy, dignity and clinical effectiveness
- Deliver evidence-based care that keeps patients safely in their own homes
- Work closely with general practice, therapy services and home visiting colleagues
- Provide guidance and support to junior staff
- Assist the District Nurse and Senior Community Nurses in managing the healthcare environment
- Travel across a wide geographical area, often carrying equipment
- Use a suitable vehicle for business purposes
