Island City Network is seeking a Social Prescriber to provide non-clinical, one-to-one support to patients facing loneliness, housing, money, employment or caring challenges. The role involves mainly telephone contact with some face-to-face appointments and community outreach, working with GPs and care teams.
You will complete NHS England Social Prescribing certification and Personalised Care Institute online learning, build trusted relationships, and help patients access local services.
