Staff Nurse post in the East District Nursing team, based at Ledbury for Wye Valley NHS Trust, covering 30 hours a week. The work centres on housebound patients in the East Primary Care Network, helping them manage their own care and supporting people and families at the end of life alongside hospice teams. The role also includes a District Nurse clinic for non-housebound patients and close links with GPs, care homes and care agencies. Applicants need RGN registration and experience of teaching within clinical areas.
What the job involves
- Provide district nursing care to housebound patients in the East Primary Care Network
- Run a District Nurse clinic for non-housebound patients who need specialist District Nurse skills
- Support patients towards self-management through education, equipment and lifestyle changes
- Support patients and families at the end of life, liaising with hospice teams and specialist palliative care nurses
- Work with GP colleagues, multi-disciplinary staff, care homes and care agencies
- Record all work electronically on EMIS, which links to GP services
Qualifications
- Teaching qualification or course relevant to speciality, or studying towards one
- Evidence of relevant post-registration training
