This permanent, full-time Level 1 social worker post sits in the Hospital Social Work Team at Lincoln County Hospital and is aimed at newly qualified and early career practitioners. The work centres on Care Act 2014 assessments, care and support planning and reviews, plus safeguarding enquiries and joint working with health colleagues to deliver safe hospital discharges. The team also covers hospitals across northern Lincolnshire and Goole, with a weekend and bank holiday rota of one weekend in five and hybrid working. Candidates should be comfortable with a criminal record check and in spoken English.
What the job involves
- Manage a caseload under supervision, building professional confidence and autonomy
- Carry out Care Act 2014 assessments, care and support planning and reviews
- Work with individuals, families, carers and professionals to identify strengths and promote independence
- Contribute to safeguarding enquiries, keeping the person's voice and desired outcomes central
- Work with health colleagues to support safe and timely hospital discharges
- Identify wider needs such as housing, health or community support and work with partners to meet them
- Promote strengths-based practice and help people build and maintain support networks
