Livewell Southwest is seeking a Band 6 physiotherapist to join the Discharge to Assess Team. You will deliver advanced assessments and interventions for patients with complex needs at home and in care homes, working within a fast-paced multidisciplinary team to support timely discharge from acute hospital services.
The role involves leading on practice changes, supervising staff and students, and contributing to service development and audits. A 37.5-hour week with 7-day service is expected.
