Gloucestershire Health and Care NHS Foundation Trust is seeking a part-time (18.75 hours/week) nurse to join the Home First service. The role focuses on holistic assessments, care planning and safe discharge planning for patients returning home from hospital, working within home and community settings alongside GPs and therapists.
You will support medicines management, health coaching and collaborative care across MDT, with supervision and leadership to colleagues and students as needed.
