St Peter & St James Hospice in East Sussex seeks a Rehabilitation Support Worker to deliver rehabilitative palliative care under physiotherapists and occupational therapists. You will help patients reach personal goals in mobility, daily living and breathlessness, including home visits and group sessions at the Living Well Centre.
You will work within a multidisciplinary team, support carers, maintain records and coordinate workload.
