This Primary Care Nurse post sits in the primary care team at HMP Drake Hall, a women's closed prison in Staffordshire run by Practice Plus Group. The work centres on a nurse-led service aimed at preventing hospital admissions, covering medication rounds, controlled drug dispensing, emergency response and patient assessment. It is a full-time role on a 12-week rolling rota that includes weekend shifts. Applicants need adult NMC registration and must pass additional vetting and security checks.
What the job involves
- Work within a multi-disciplinary team to deliver a nurse-led service and prevent hospital admissions
- Undertake medication rounds
- Dispense controlled medication
- Attend emergency response situations
- Assess patients
- Undertake reception screening
- Deliver nurse-led clinics to the patient group with multi-disciplinary support
- Provide evidence-based care, seeking and acting on patient feedback
- Deliver non-judgemental care
- Build strong links with partners and stakeholders
- Promote best practice, clinical supervision and continuous quality improvement
- Share knowledge and skills to ensure safe care
