The Nursing Associate post sits within the Meridian Primary Care Network, a group of six GP surgeries across Cambridgeshire and North Hertfordshire, and works under the Frailty Lead. The role centres on nursing care for frail, older and clinically complex patients in their own homes, care homes and primary care settings, with a focus on preventing deterioration and avoidable hospital admissions. It is a multidisciplinary post with close links to GPs, community nursing, therapists and social care, and lists full-time, part-time, flexible and home working. Fit depends on NMC registration as a Nursing Associate and having own transport for regular travel across the locality.

  • Identify early signs of deterioration and escalation concerns promptly to the Frailty Lead or appropriate clinician
  • Support routine and proactive clinical reviews in care homes and support care home staff with observations and care planning
  • Prevent avoidable admissions through self-management support, signposting and clinically safe community interventions
  • Deliver health promotion on falls, nutrition, hydration, mobility, vaccination and screening awareness, and smoking cessation
  • Communicate sensitively with patients, relatives and carers, including those with cognitive or hearing impairment, and liaise across health and social care
