The Reconnect Care after Custody service, run by Lancashire & South Cumbria NHS Foundation Trust, supports people leaving six prisons. The practitioner triages, assesses and refers them from up to three months before release, with support for up to six months after.
Work is with people facing complex needs such as homelessness, substance misuse and mental health, with hybrid working across Trust sites and home. Applicants must hold the right to work in the UK.
