The role focuses on medication reviews, medicines reconciliation, safe prescribing, and repeat prescribing, with independent prescribing and guidance from a Senior Clinical Pharmacist.
You will handle medicines queries from patients and the wider team, manage chronic disease care (hypertension, type 2 diabetes, asthma, COPD)
