This post sits in the community nursing team of County Durham and Darlington NHS Foundation Trust, with vacancies across the county. What the job involves
- Provide nursing care to patients in their own home or living environment
- Administer insulin
- Carry out basic wound care
- Collect samples, including by venepuncture
- Record clinical observations and changes in patients' physical and social condition
- Provide general care and support to patients and their families
- Carry out care scheduled by a registrant
- Work alone with patients within an agreed care plan
Qualifications
- NVQ Level 3 or Level 3 Diploma in Health & Social Care, or equivalent relevant experience
- GCSE equivalent in English and Mathematics
- Knowledge of Microsoft packages
Experience
- Experience of working within multidisciplinary teams
- Experience of working with adults
- Relevant assistant experience
Skills & knowledge
- Communicate clearly and effectively
- Observe and record physical and social changes in patients
- Identify when to report to a registered nurse or therapist
- Work with minimal supervision within an agreed care plan
- Undertake basic clinical activities with training, such as venepuncture, simple wound dressings and blood glucose monitoring
- Understanding of multidisciplinary and inter-agency working
