HCRG Care Group is seeking a Community Nurse (Band 5) to deliver patient-centered care in patients' homes around Melksham. You will assess needs, plan and deliver nursing care, and monitor changes in condition with a multidisciplinary team.
The role is 37.5 hours per week, requires NMC registration and a full UK driving licence with access to a car for work, and you will maintain detailed records and contribute to quality improvement.
