Haringey GP Federation and Whittington Health NHS Trust are seeking a GP to provide clinical input for weekly Complex Proactive MDTs focused on frailty and long-term conditions within the Integrated Neighbourhood Team model.
The role supports MACC neighbourhood teams and acts as a primary care link across the INT network, working with clinicians and community partners to promote preventative, person-centred care across boroughs.
