Armley PCN's Proactive Care Team is recruiting a healthcare assistant to support frail patients, mostly at home, across three GP practices. The role includes frailty reviews, low-level clinical screening and support for care plans, with tasks such as phlebotomy and blood pressure checks.
Driving licence and PCI course or willingness to complete are required. You'll work across practices and community sites, with flexible hours (full-time, part-time or compressed).
