Hospice in the Weald is seeking a Clinical Nurse Specialist or Clinical Paramedic Specialist to join the Hospice Outreach Service. The post-holder will be the first contact for new community referrals, providing palliative advice by phone and carrying out clinic and home assessments on a seven-day rota.
Clinical duties include delivering medicines on visits, guiding primary care teams, supporting carers, recording histories and presenting cases at MDT meetings, with opportunities to mentor
