Kent Community Health NHS Foundation Trust is seeking a Healthcare Assistant to support housebound adults with complex, long-term conditions in their own homes. The role involves delivering wound care, medication support and bladder/bowel care, working mainly alone and planning seven-day visits from 08:00 to 18:00.
You will participate in holistic assessments, care planning, daily handovers and MDT collaboration, signposting to other services and supporting patients to maintain independence.
