James Paget University Hospitals NHS Foundation Trust is seeking a Patient Flow Support Worker to join the Patient Flow team. You will help coordinate discharges, transfers and repatriations, liaising with ward teams and external partners to progress patient pathways and address delays.
You will use OPTICA and WardBoard to monitor pathways, support complex discharge needs, and act as a contact for discharge planning and escalation. GCSEs in English and Maths are required.
