Elm Lodge Surgery is seeking an experienced nurse to join its multidisciplinary team. The role focuses on holistic geriatric assessments for frail older patients, conducted either at patients' homes or in the surgery, with subsequent care planning and referrals as needed.
Applicants should be an NMC-registered nurse with experience in multidisciplinary settings; primary or geriatric care is a plus. Training in geriatric assessment will be provided.
