The role focuses on delivering intensive, evidence-based community care, preventing inpatient admissions and supporting young people with meal-related and psychosocial interventions.
You will work within a multidisciplinary team across in-reach, outreach and day hospital settings, contributing to care planning and discharge support with families and carers.
