South London and Maudsley NHS Foundation Trust is recruiting a Care Coordinator to join the Lewisham CMHT, working with nurses, psychiatrists and other professionals to support people with complex mental health needs in the community.
You will formulate flexible care plans with service users and carers, conduct dynamic risk assessments and work collaboratively within the team. Relevant registration and substantial clinical experience in mental health are essential.
