Hedena Health, a 25,000-patient GP practice, is recruiting a six-month community health and wellbeing worker to help people of all ages make healthier choices. The work is mainly field-based: regular home visits to up to 120 households, linking people with GPs, social workers and charities. • Identify problems and needs and help find solutions

  • Support health coaching and social prescribing, helping people set and reach goals
  • Work with GP surgeries, doctors, nurses, social workers and charities to support vulnerable people
  • Encourage attendance at health checks, vaccinations and screenings
  • Build relationships with local community groups and volunteers, and support their work
  • Help people understand and use health and social care services and manage their conditions
