A six-month Enhanced Access clinical pharmacist post with the LMA West London GP Federation, supporting general practice in its area. The work is episodic rather than continuity-based: the pharmacist assesses each patient's presenting need, covering medication reviews, acute conditions and prescribing, then hands ongoing care to the registered practice. It sits within a multidisciplinary primary care service where safety-netting and clear clinical records are central. Full GPhC registration with no restrictions is required; independent prescribing is desirable.
