The Band 5 occupational therapist post is based on Jasmines Ward, an older people’s inpatient ward within the South West London and St George’s Mental Health NHS Trust. You will manage a defined caseload, deliver targeted OT interventions, and support recovery goals both in hospital and during home visits.
Key duties include discharge planning, collaboration with the multidisciplinary team, supervision of the activity coordinator, and contributing to care pathways from admission to discharge.
