HT Practice, a group of four GP practices, is recruiting an Advanced Nurse Practitioner to work across its sites and carry out home visits. The post centres on patient assessment, care planning, prescribing and monitoring, with all clinical contacts recorded electronically.
Hours can be full or part time, finishing at 18:30 with occasional later working for urgent need, and training includes weekly tutorials and on-call GP cover.
