About the opportunity
The Care Partner role is a full-time position supporting clients in the Gippsland Bass Coast South Gippsland and Latrobe regions. In this role, you will be responsible for managing and supporting clients whilst also identifying opportunities to expand our Home Care service support to the community. You will play a key role in ensuring high-quality, person-centred care in an empathetic caring manner.
What you’ll bring
- In depth knowledge of the current Support At Home model of care
- Minimum 2 years’ experience in aged care case management, aged care services coordination and or disability.
- A relevant qualification in community services or case management that has been attained or currently being acquired.
- Excellent time and client budget management, organisational and communication skills, with the ability to adapt and problem solve.
- A compassionate, flexible and positive approach to effective patient care and management.
- The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.
- The ability to work both autonomously and in a dynamic team environment.
You will also be required to provide evidence of, or in the process of obtaining: A current Police Record Check.
