PCN General Practice Registered Nurse in Luton

We have an exciting opportunity for a qualified experiencedGP Practice nurse to join our Primary Care Network team.

PCN Practice Nurse equipped with the knowledge,understanding, skills, attitudes, and behaviours to support general practice.The PCN Nurse will assist with the delivery of health promotion, phlebotomy,chronic disease management, immunisation and vaccinations and other clinicalroles in line with their training and level of experience. The PCN Nurse willhave a key role in supporting delivery of the QOF and Network Contract DES Service specifications

Main duties of the job

• Deliver evidence-based nursing care within primary care settings.
• Undertake chronic disease reviews (e.g. Diabetes, Hypertension, Asthma and COPD).
• Provide immunisations, vaccinations and cervical screening.
• Support health promotion and preventative care initiatives.
• Participate in quality improvement, clinical audits and service development.
• Maintain accurate clinical records and uphold professional standards.

INTRODUCTION TO THE PCN

The Medics Primary Care Network (PCN) serves apopulation of under 64 525 patients residing across the Luton in Bedfordshire.

Working collaboratively with other partners tostrengthen and support the delivery of Primary Care services, the PCN consistsof 5 core GP practices Woodland Avenue Practice, Medici Medical Practice,Gardenia & Marsh Farm Practice, Bell House Medical Centre & BartonHills Medical Group.

Medics PCN have a dedicated network site where many of the MDT, personalised care team and clinicians work collaboratively to deliver a pro-active approach to patient care.

Job responsibilities

Key Responsibilities and Duties:

Work within the PCN to provide and monitor GeneralPractice Nursing care and Long term conditionsmonitoring alongside the wider PCN team.contributing to the delivery of excellent patient care, supporting the existingworkforce under direct or indirect supervision.

Improve safety and quality of care at every opportunity.

Undertake additional clinical training asrequired by the PCN to support QOF and the PCNNetwork Contract/DES Services Specification

Support all clinical and non-clinical staff andthe wider team

To support with chronic disease clinics andmaintaining registers, deliver patient care, referring patients to othermembers of the PCN or to secondary/specialist care as appropriate. To implementand evaluate individual treatment plans dependant on training and experience.

Provide support to the PCN and Practices withregards to their QOF and Network Contract DES Services Specification includingCVD prevention and tackling health inequalities

Develop relationships across the PCN to supportintegration of the role across health and social care: including communitycare, secondary care, and mental health services.

Provide support and supervision to traineenursing associates, healthcare assistants, apprentices, and those on learningplacements appropriate to level of competence.

Accurately perform and record clinicalobservations such as blood pressure, temperature, respiration, and pulse rates.

Engage positively with patients/carers/familiesand staff members promoting mutually respectful and appropriate relationships.

Communicate effectively with patients regardingproviding and planning their care and carrying out risk assessment needs as required.

Communicate complex and sensitive informationverbally and in writing.

Adapt and tailor communication skills to theneed of the listener.

To act as patient advocate during practice orPCN meetings, sharing any relevant information confidently and accurately,maintaining patient confidentiality appropriately

Develop an understanding of MDT structures andmembers roles.

Support patients to improve and maintain theirphysical, mental, and behavioural health and well-being.

Provide advice to patients including promotingself -management where appropriate and signposting patients to other PCNcolleagues, local community, and voluntary sector services.

Maintain accurate and contemporaneous patienthealth records.

Skills and training need to be updated throughcontinuous professional development, attending regular educational updates, viae-learning and face to face sessions as appropriate

Wound management, undertake dressings and sutureand clip removal.

Manage long term conditions such as diabetes,asthma, COPD and hypertension.

To implement and evaluate individual treatmentplans for chronic disease patients that promote health and wellbeing.

To identify, manage and support patients at riskof developing long term conditions, preventing adverse effects to patientshealth.

As required to provide routine nursing care topatients in accordance with clinical based evidence, NICE Guidelines.

To oversee the provision of chronic diseaseclinics, delivering patient care as necessary, referring patients tosecondary/specialist care as required.

To ensure the appropriate maintenance and use ofchronic disease registers.

To ensure compliance with PCN policies andprocedures.

To support the team in dealing with clinicalemergencies.

To implement vaccination programmes for adultsand children.

To support the clinical team with allsafeguarding matters, in accordance with local and national policies.

To provide support to Practices in the PCN withmanagement of chronic diseases such as Diabetes, COPD, Asthma, Hypertension, byproviding clinical sessions.

Experience

• Experience in health and or social care.
• Communication and relationship building.
• Experience of developing relationships with a wide variety of people.

• Experience of health and social care assessments.
• Demonstrated successful outcomes delivery within determined timeframes.

Qualifications

• Registered Nurse with the NMC With General Practice Experience and Chronic Disease checks experience.
• Knowledge of GP QOF.
• Demonstrable commitment to professional and personal development.
• Basic/advanced certificate of life support.
• Awareness of the Transforming Nursing plan
