Female Community Support Worker

As a Female Community Support Worker at our Learning Disability Nursing Home, you'll play a key role in helping the people we support live active, fulfilling and connected lives. You'll lead and coordinate a range of meaningful community activities, one-to-one support and group programmes, helping individuals build confidence, develop skills, pursue their interests and strengthen social connections. From organising day trips and local activities to supporting people to achieve their personal goals, you'll ensure that each person has opportunities that reflect their individual choices, needs and aspirations. You'll also provide person-centred personal care and practical support where required, always promoting independence, dignity, choice and wellbeing.

As part of the role, you'll safely drive the service's 7-seater minibus, enabling individuals to access activities and opportunities within their community. You'll be responsible for routine vehicle checks and associated documentation. Working closely with colleagues, families, community organisations and other professionals, you'll help create positive experiences and opportunities that make a meaningful difference to the lives of the people we support.

Hours and Pay

• Hours: 16-20 hours on a rota basis, across 7 days a week.
• Pay: £13.18 per hour.

Key Responsibilities

• Support individuals to access meaningful community activities, pursue personal interests and develop independence, confidence, life skills and social connections.
• Plan, organise and deliver person-centred one-to-one and group activities, outings and community events based on individual needs, preferences and aspirations.
• Build and maintain positive relationships with community groups, local services and recreational providers to expand opportunities for participation and inclusion.
• Promote choice, dignity, wellbeing and active involvement in decisions about activities and support.
• Provide personal care and practical support in accordance with individual care plans, assessed needs and agreed support arrangements.
• Support the coordination, monitoring and evaluation of activities, contributing to activity planning and continuous service improvement.
• Safely drive service vehicles, completing required safety checks, mileage and fuel records, and promptly reporting defects or maintenance requirements.
• Promote individuals' safety and wellbeing, following safeguarding procedures, risk assessments and relevant organisational and regulatory requirements.
• Maintain accurate records, activity documentation and clear communication with individuals, families, colleagues and external professionals.
• Develop positive, professional relationships while maintaining confidentiality and appropriate professional boundaries.
• Participate in team meetings, supervision, training and continuous professional development.
• Uphold organisational values and promote equality, diversity and inclusion in all aspects of the role.
• Undertake any other duties reasonably required within the scope of the role.
