At CCH Group, we deliver 1.5 million hours of care monthly across 260+ locations. As a Care Coordinator, you will be instrumental in organizing and delivering high-quality care services right where it matters most.
Location: York, YO30 5PB
Salary: ÂŁ25,364 + additional payments for on-call duties
Driving License: Required
Working Hours: Monday to Friday, 9 AM - 5 PM, with additional on-call duties on a rota basis
What You’ll Do:
- The Care Coordinator plays a crucial role in ensuring the highest quality of care services by managing care workers and performing field-based duties. Your responsibilities will include:
- Assessing and reviewing individual care plans
- Supervising, instructing, and supporting care staff in the field
This role requires a valid driver’s license and access to your own vehicle due to the travel involved.
