The Service is designed to deliver end of life and respite care for patients with specialist palliative care needs that wish to remain in their own home. Care is delivered in the patients' home by Health Care Assistants.

Main duties of the job:

  • The post holder will be responsible for the management of the St Johns Hospice @Home service which provides home care for patients living within our catchment area of Westminster, Kensington, Chelsea, South Brent and Hammersmith and Fulham.
  • The post holder will be responsible for the overall management of the service.
  • You will work with two clinical support nurses and a team of Health Care Assistants.
  • You will be supported in your role by the Community Specialist Palliative Care team lead.

Job responsibilities:

  • To maintain confidentiality surrounding patients on the caseload.
  • To ensure documentation is completed accurately and within agreed timescales using Hospital approved documentation, in line with NMC and Hospital guidelines.
  • To have knowledge of and adhere to Hospice and Hospital policy and professional standards. To ensure that team members do likewise.
  • To demonstrate a professional approach to work, and act in accordance with the Hospital Code of Conduct and the NMC Code of Professional Conduct at all times.
  • To participate in staff appraisal, staff development and in-service training activities.
  • To meet Revalidation requirements.
  • To identify own learning needs and ensure own professional development is maintained by keeping up to date with practice developments.
  • To undertake further training and academic qualifications as relevant to the role and service requirements.
  • To seek opportunities to develop the service.
  • Ensure that appropriate and timely data is complete to allow reports to be sent to commissioners.
  • To ensure that all leave, whatever the reason, is managed appropriately.
  • Ensure all regulated activity is managed appropriately.

Person Specification:

Experience:

  • Significant experience of working in Palliative Care either in the community or an inpatient care setting.
  • Experience of working with members of a multi-disciplinary team.
  • Experience of coordinating with external teams e.g. Continuing Health Care, Community Nursing.
  • Understanding and commitment to the values of our organisation.
